📄 DescriptionA 36‑year‑old man in Hangzhou developed vertebral artery dissection and stroke after using a massage gun on his neck for 3 days. Doctors warn: neck, eyes, chest, armpits are strictly forbidden – may cause blindness, organ damage or stroke. Safe only on large muscle groups.
📌 AbstractMassage guns are not “simple massagers”. The neck's delicate vessels can be torn by high‑frequency vibration, triggering stroke. Eyes, chest, abdomen, and armpits are danger zones. Only use on thick muscles (glutes, hamstrings, calves) for 1‑2 min, low intensity, moving. Contraindicated for osteoporosis, pregnancy, heart disease, etc.
Massage gun warning

A 36‑year‑old man in Hangzhou, China, used a massage gun on his neck for 3 consecutive days due to discomfort. Days later he suffered sudden dizziness, nausea, vomiting, and fell into a coma. Rushed to hospital, he was diagnosed with vertebral artery dissection causing a stroke. After surgery, he recovered.

The massage gun was once hailed as a “miracle massager” for any pain – not only for relaxation but also labelled as “must‑have for athletes”, “cure for neck pain”, and even “treatment for diseases”. But is it really that magical? Why can it cause a stroke? Let's dive in.

Massage gun usage

A massage gun, technically a deep muscle fascia impact device, can help relieve myofascial pain and promote muscle recovery. However, if applied to blood vessels, nerves, internal organs, or the eyeball – which are fragile – the consequences can be catastrophic.

⚠️ Massage gun on the side of the neck – risk of stroke

The neck is one of the most dangerous areas. It contains major blood vessels, nerves, and respiratory/digestive passages – as vital as the head and heart.

Neck anatomy vessels and nerves

For flexibility, the neck is mainly covered by muscles, skin, and fascia – no heavy bones or thick fat for protection.

Many neck vessels are superficial. For example, the common carotid artery can be felt pulsating on both sides of the trachea.

⚠️ The intima of these vessels is extremely delicate. High‑frequency violent vibration from a massage gun can easily tear the vessel wall (vertebral artery dissection). Clots from the tear can travel to the brain and cause a stroke.

Medical literature documents a case of a young woman who suffered ischemic stroke (brain infarction) after using a massage gun on her neck, eventually requiring tracheostomy.

Skin redness, bruising, muscle strain, or joint injury are the mildest outcomes.

⚠️ The high impact on the head and neck can also travel to the inner ear, shaking loose the otoliths, causing vertigo and nausea.

An observational report found that some people developed benign paroxysmal positional vertigo (BPPV) after using a massage gun on the neck and shoulders; one patient with recurrent BPPV had no recurrence after stopping the gun.

👁️ Massage gun on the eye – risk of cataract or blindness

The eyeball is far more fragile than muscle. The impact on the eyelid transmits to the lens and retina. The eye is essentially a delicate water balloon; the high‑frequency impact (dozens of hits per second) is comparable to shooting an air gun at close range.

❌ On the front of the eye: corneal abrasion, edema, massive intraocular bleeding, pupil dilation failure, traumatic cataract and glaucoma.

❌ On the back of the eye: retinal concussion, edema, haemorrhage or even perforation (macular hole), causing sudden vision loss.

❌ Worst case: the eyeball wall is ruptured, and the contents explode out – possibly requiring surgical removal of the eye.

An open case report described a male college student in his 20s who used a massage gun weekly for eye fatigue, resulting in tears and damage to both the outer layer and the fundus.

Doctors liken the vibration to violently shaking a raw egg – causing lens dislocation and retinal rupture.

More dangerously, eye injuries often have a delayed onset – symptoms appear days or even weeks later, easily missing the optimal treatment window.

Massage gun on chest and abdomen – risk of vessel and organ damage

Many people, while massaging their back, move the gun to the stomach or chest – some even use it on the abdomen as a weight‑loss “hack”.

Under the chest are the heart and lungs; under the abdomen are the liver, stomach, intestines, and major vessels and nerves. High‑intensity vibration and impact can injure soft tissue, blood vessels, nerves, and even internal organs. Those who have had abdominal surgery, hernias, or are very thin should avoid using it on the chest and abdomen.

🦵 Massage gun on armpit, elbow pit, knee pit – nerve and vascular injury

These areas look tough but are actually vulnerable. Dense with nerves and vessels, they are protected only by a thin layer of skin. Direct impact can cause redness, swelling, or even nerve damage – like an electric shock.

Certain groups have lower tolerance – vibration can cause irreversible damage. People with osteoporosis, pregnant women, diabetics, cancer patients, and heart disease patients are advised not to use massage guns.

✅ Where CAN you use a massage gun?

Many think it's a "no‑brainer" massager – wrong. Its physical threshold and safety boundaries are much narrower than imagined.

Here's a simple rule:

✅ You can pinch a thick lump of muscle – consider it.

❌ If you feel bone, obvious vessels, or vital organs underneath – don't use it.

Relatively safe areas are large, thick muscle groups: glutes, hamstrings (back of thighs), and calves (gastrocnemius). These are the true home ground for a massage gun.

When using, follow these details:

  1. Stay on each area for 1‑2 minutes, low intensity, move slowly – do not linger on one spot.
  2. If you have acute injury, inflammation, history of thrombosis, or osteoporosis, consult a doctor before use.
  3. If you are on anticoagulant/thrombolytic/antiplatelet drugs, or have pulmonary bullae, aortic aneurysm, spleen disease, etc., consult a doctor before use.
  4. If you experience persistent pain, numbness, dizziness, or vision changes after use, seek medical attention immediately.
